Healthcare in Britain

The National Health Service (NHS) is a source of immense national pride in Britain, providing free healthcare at the point of use to all residents. To access these services, your first step is always to register with a local General Practitioner (GP). For non-urgent medical issues, your GP is your main point of contact, while serious emergencies are handled at the hospital's A&E (Accident & Emergency) department. Understanding how the system works—and the specific vocabulary involved—will help you navigate appointments, get prescriptions, and receive the care you need confidently.

Describing Duration of Illness (For vs. Since)

When visiting a doctor in the UK, they will almost always ask how long you have been experiencing your symptoms. We use the Present Perfect tense with 'for' and 'since' to describe conditions that started in the past and continue into the present.

Common Mistakes to Avoid

✓ How long have you had these symptoms?

✗ Since when do you have these symptoms?

At the GP Surgery

Doctor: Good morning. What seems to be the problem today? Elif: I've had a really bad sore throat and a temperature since Sunday. Doctor: I see. Have you had any other symptoms — a cough or difficulty swallowing? Elif: Yes, it's quite painful to swallow. I've also been feeling a bit under the weather in general. Doctor: How long have you had the temperature? Elif: For about three days. I took paracetamol, but it keeps coming back. Doctor: Let me have a look. Open wide... Yes, it looks like tonsillitis. I'm going to prescribe a course of antibiotics. Elif: Do I need a referral to a specialist? Doctor: Not at this stage. If it doesn't clear up in a week, come back and we'll reassess. Elif: I also need to give my employer a sick note. Can you provide one? Doctor: You can self-certify for the first seven days, but I'll print one out for you just in case. Elif: Thank you. Where do I get the prescription? Doctor: Any pharmacy. There's one in the high street just opposite the walk-in centre.

British Culture: The 'Stiff Upper Lip' and the NHS

In British culture, people often downplay their illnesses. If someone has a terrible flu, they might simply say they are feeling 'a bit under the weather.' Complaining too much about minor ailments is traditionally frowned upon, reflecting the famous British 'stiff upper lip.' However, complaining about NHS waiting times is practically a national pastime! Despite the grumbling, the British are fiercely protective of the NHS, viewing it as a cornerstone of their society. And of course, no matter what the illness is, a Brit will usually offer you a cup of tea to make you feel better.

A letter from a GP surgery

Dear Mr Yılmaz, Following your recent appointment, Dr Ahmed has made a referral to the dermatology department at St Luke's Hospital. You will be contacted directly by the hospital with an appointment date, usually within six to eight weeks. If you have not heard within eight weeks, please contact the surgery rather than the hospital. Please note that if you are unable to attend, you must give at least 48 hours' notice to reschedule. Patients who miss two consecutive appointments without notice may be referred back to their GP. There is no need to contact us in the meantime unless your symptoms worsen significantly.
